Diabetes: Invitation to a Healthier Lifestyle

The Portland Clinic has designed this five-weekseries specifically for people with Type 2 diabetes. Whether you have been diagnosed recently, or you've been living with diabetes for years, we encourage you to learn more about diabetes self-management so you can take control of your health.

 

In five group sessions, each one hour and 45 minutes long, you'll learn to:

  • Understand your diabetes
  • Use nutrition to manage diabetes
  • Plan meals
  • Follow a healthy eating plan away from home
  • Check your own blood sugar the quick and easy way
  • Understand exercise and its effects on diabetes
  • Prevent diabetes complications

 

Each session is taught by a registered nurse and a registered dietitian/certified diabetes educator. Your instructors will work as a team with you and your doctor or nurse practitioner to make sure you receive both personal attention and professional guidance. Our goal is to help you gain the skills, knowledge and confidence to maintain your good health.

 

Fee: Many insurance plans cover the cost of this series. Contact your insurance company

directly to ask about coverage for "diabetes self-management education." This series is a fully covered benefit for members of Secure Horizons and Providence Medicare Extra whose primary care provider is at The Portland Clinic.
