Anticoagulation Clinic

Overview

Anticoagulants — such as warfarin, Pradaxa, Xarelto and Eliquis — are blood-thinning medications that can help prevent blood clots. While these medications provide important benefits, they need to be monitored closely to avoid complications.

The Portland Clinic’s anticoagulation clinics are staffed by registered nurses who have special training in anticoagulation therapy. They work closely with patients and their health care providers to help manage this therapy safely. To do this, they meet with you periodically to take a quick finger-stick blood test, and to adjust your medication dosing if needed, based on the results. They also monitor you for any changes in your diet, health or other medications that could affect your anticoagulant regime. In addition, they work closely with your other providers to ensure your safety if you need to have surgery, a biopsy or another procedure.
