Before your Botox procedure, we want you to have a clear understanding of your potential costs. The information below answers common questions about procedure estimates, insurance coverage, self-pay options, and payment.

Why am I receiving an estimate for my Botox procedure?

As part of our commitment to help you understand the cost of your care, The Portland Clinic will provide you with a good faith estimate that represents our best effort to forecast the potential costs of your Botox treatment based on the information available on the date it is issued.

Is this estimate my final bill?

No. Your estimate is based on the information available at the time it is prepared, including your insurance benefits and the planned procedure. Your final balance may be different depending on the services you receive and how your insurance processes the claim.

What may cause my final cost to vary?

Health care is complex, and sometimes your care needs can change. Your final cost may differ from the estimate if:

  • Your provider determines that a different amount of Botox is required or additional services are medically necessary during your visit.
  • Your insurance benefits, deductible, co-insurance, or copay change.
  • Your insurance processes the claim differently than anticipated.
  • Additional procedures or treatments are performed during your appointment.

Does insurance cover Botox injections?

Coverage for Botox injections varies by insurance plan. While Botox may be covered for certain medical conditions, you are responsible for any applicable deductibles, copay, co-insurance, or services not covered by your insurance.

What if my insurance changes before my appointment?

Please notify us as soon as possible if your insurance coverage changes before your procedure. A change in insurance may affect your estimated and final out-of-pocket costs.

When will payment be due?

Any estimated patient responsibility is due prior to your in-office procedure. Once your insurance company reviews and processes the claim, your actual financial responsibility may be higher or lower than the estimate provided. You will be billed for any additional balance due or refunded if overpayment has occurred.

What if I choose to self-pay for my procedure?

If you are not using insurance, we offer a clear and straightforward payment process to help you plan ahead.

  • 50% deposit is required at the time you schedule your procedure.
  • The remaining balance is due 72 hours prior to your procedure.
  • 20% discount will be applied to the remaining balance when it is paid in advance.

This approach is designed to provide transparency and help reduce financial uncertainty, so you can focus on your care.
