The Portland Clinic is contracted with several Medicare Advantage plans. These insurance plans offer full Medicare benefits plus extra benefits beyond Original Medicare. Patients on Original Medicare are highly encouraged to sign-up with a Medicare Advantage plan. If you’re a current patient and on a Medicare Supplement plan, you can continue to be seen at The Portland Clinic. Please note, our primary care providers no longer accept new patients with Original Medicare.

SeniorChoices NW

The Portland Clinic has a partnership with SeniorChoices NW, a local, licensed insurance agency specializing in Medicare-approved insurance plans, to assist you in selecting the plan that best fits your needs. There is no cost for their service. We encourage you to call them to schedule an appointment.

  • Go online to SeniorChoices NW (www.seniorchoicesnw.com) and click on “Scope of Appointment Form” in the top navigation bar on the site.
  • After you complete the Scope of Appointment Form*, complete the Request Appointment Form.

*Please note: Medicare requires all Medicare beneficiaries complete a Scope of Appointment Form at least 48 hours prior to any plan discussion. Please keep this in mind when scheduling.
