The Portland Clinic honored for achievements in diabetes care

AMGA recognizes top performers in national diabetes initiative

AMGA Awards

In March, The Portland Clinic was honored with an award to celebrate the clinic’s achievements in diabetes care. The AMGA Foundation honored seven organizations across the nation with Together 2 Goal® Awards, recognizing the top performers in the AMGA Foundation’s national initiative, which united 150 medical groups and health systems in an effort to improve care for 1 million people with Type 2 diabetes by 2019.

In conjunction with the campaign, doctors and nurses at The Portland Clinic implemented best practices to improve the quality of diabetes care for our neighbors, reporting tremendous results for diabetes patients on a variety of measures, including blood sugar control, blood pressure control, lipid management, and testing for kidney disease for residents of the Rose City.

The Portland Clinic has made significant progress in diabetes care since 2016, including cleaning up our data to ensure reports accurately reflect performance, and prioritizing measures with the greatest impact on improving patient outcomes. In 2018, The Portland Clinic will focus on improving disease registries which will help provide comprehensive and evidence-based diabetic care, and better support our providers and nursing staff.

In 2017, The Portland Clinic observed the following outcomes among its patients with diabetes:

  • Type 2 diabetes prevalence decrease by 2 %
  • HbA1c control increase by 4.7%
  • Blood pressure control increase by 3.9 %
  • Medical attention to nephropathy increase by 11.6%
  • Lipid management increase by 4.8%
  • Diabetes care bundles increase by 8.6%

It’s an honor to be recognized for the care we provide to our diabetic patients and the continued commitment our doctors and nurses keep to help ensure our neighbors in Portland remain healthy. The Portland Clinic invests in our staff, our community and the wellness of each patient, and we are extremely thankful for those patients who have committed to manage and prevent this chronic condition. This award is a testament to that dedication, and we are very proud of this national recognition.
