Overview

The Portland Clinic Diabetes Center for Comprehensive Care provides patients with the specialists, expert consultants and classes to effectively manage the disease of diabetes. We’re proud to offer a personalized health care program that is tailored specifically to the needs of individuals with diabetes.

SERVICES (including, but not limited to)

• Team approach involving primary care providers, certified diabetes educators, registered dietitians and registered nurses
• Continuous glucose monitor management and insulin pump management
• Planned, longer health care appointments
• Opportunity for telehealth follow-up
• Coordination of care with your primary care provider and advanced practice provider

NOTE:  Starting in mid-December, our Downtown location at 800 SW 13th Avenue will begin closing to patient care and all clinical operations will be relocating to our five neighborhood locations in the Portland Metro area. Downtown diabetes services will transition to our Beaverton and Northeast clinics. Read more here >>

HOW TO PREPARE FOR YOUR DIABETES SERVICES APPOINTMENT:

• How can I schedule an appointment?
  • Request a referral from your primary care provider. If you have a referral and have not heard from the team in two weeks, please call 503-221-0161 ext 2327 to schedule your appointment.
    
• What should I bring to my appointment?
  • Your continuous glucose monitor (CGM) or blood glucose meter. If you use a mobile app to track your readings, please have your device ready.
  • Your insulin pump, if you use one.
  • A list of your current medications, including doses.
  • Any questions or concerns you have about your diabetes care. 

• What should I expect at my first visit?
  • There will be a comprehensive assessment during your first visit, Which may include:
  • Reviewing your diabetes history and treatment plan
  • Checking for diabetes complications and other health conditions
  • Discussing your daily routine, eating habits, physical activity and sleep
  • Talking about your support system and any challenges you face in managing diabetes
  • Reviewing your used of diabetes technology (CMG, insulin pump, apps)
  • Setting goals together for your diabetes care
