Overview
The Portland Clinic Diabetes Center for Comprehensive Care provides patients with the specialists, expert consultants and classes to effectively manage the disease of diabetes. We’re proud to offer a personalized health care program that is tailored specifically to the needs of individuals with diabetes.
SERVICES (including, but not limited to)
- Team approach involving primary care providers, certified diabetes educators, registered dietitians and registered nurses
- Continuous glucose monitor management and insulin pump management
- Planned, longer health care appointments
- Opportunity for close phone follow-up
- “Diabetes pathway” system that targets your health care needs
- Coordination of care with your primary care provider and advanced practice provider
NOTE: Starting in mid-December, our Downtown location at 800 SW 13th Avenue will begin closing to patient care and all clinical operations will be relocating to our five neighborhood locations in the Portland Metro area. Downtown diabetes services will transition to our Beaverton and Northeast clinics. Read more here >>
